اتكلمنا قبل كدة عن ازاى نعمل tight contact واى الtools ال تساعدنى فى كدة
كلامنا النهارده عن ال cavity design for composite
•Types of cavity
1-traditional cavity (G.v Black)
2-saucer shape cavity
3-proximal slot prep
وانواع كتير لحد ماعملوا
General Guideline for durable restoration عشان تقدر تعمل cavity design محترم بعد ماتشيل ال caries فى 5 حاجات ابص عليهم
فنجان قهوتك وركز معايا عشان مهمين جدا
1-floor of the cavity
Flat or irregular
مافيش مشكله عندى ف الاتين لكن ال irregular افضل عشان احافظ على (RDT) Remaining Dentin Thickness
2-internal line angle
لازم تكون rounded
عشان ال sharp بيعمل stress concentration ومع ال sharp walls واحط bond بيحصل air bubbles تضغط على ال dentinal tubules تعمل pain بعد كدة
فاجيب round finishing stone واعمل roundation محترم
3-cavo surface margin or tooth restoration interface (TRI)
عباره عن نقطه التقاء بين ال restoration مع نهايه الكافيتى
لازم تكون finished عشان تسهل ال composite adaptation وتمنع ال white line
لازم تكون flared wall مايكونش Parallel عشان يفتح معايا ال enamell rods اكتر فيحسن الbonding ويمنع ال white line
لازم اشوف ال cavo surface angle نازل عليها direct occlusion ولا لا عشان ماياثرش على ال bond بين ال restoration& cavity marginويحصل fracture
فاجيب yellow coded burs وال tip تكون ف ال floor ومايل. بزاويه 45 على ال walls واعمل flaring
4-cusp thickness
اشوف ال remaining tooth structure واى undermind enamel لازم يتشال عشان امنع fracture
حتى لو غطيته ب composite هيتكسر لان الكمبوزيت brittle فاشيلها ب yellow coded burs
ولازم ال cusp thickness يكون 2mm عشان يعمل support withstand occlusal force
لو اقل من 2mm هعمل cuspal coverage
وهنتكلم عنها فى بوست لوحدها
5-cervical enamel
لازم احافظ عليه واعمله finishing محترم لانه اكتر مكان بيحصل فيه secondary caries وكمان يحسن ال bond و durability
فعشان كدة فى حالات class 2 اعمل pre-wedging. to preseve the cervical enamel
بس ياسيدى
اخد بس بالى من حاجة وانا شغال class1 وشيلت caries
اشوف بعدها ال marginal ridge
فى حالات ال premolars. لو لقيت ال thickness اقل من 1.6 افتحها class2
ولو فى ال molars اقل من 2mm افتحها class2
